Why do I need this evaluation?

Most insurers and many states require presurgical psychological and/or neuropsychological clearance as research indicates they can benefit one's medical treatment.

Evaluation results can be a crucial part of an effective treatment plan for presurgical preparations and best outcomes for postsurgical adjustment.

What to Expect

Prior to your evaluation, you will complete an intake packet of your medical history and background as well as other important documents regarding your care.

Dr. Adams will conduct an interview to obtain any additional information, discuss aspects of your surgical procedure, and answer any questions you may have about the evaluation. You may include a loved one during the interview. There will be additional questionnaires and/or other cognitive or neuropsychological testing depending on the type of surgical procedure.

There are no preparations required as the test is not pass/fail. Dr. Adams will send the report of results to your referring provider who will then determine if any modifications to your treatment plan are needed.

How long is the evaluation?

The consultation typically lasts between 1-2 hours. The time will vary dependent on many factors such as the complexity of your condition, personal information, and addressing any questions you may have about the evaluation.

What do I need to bring to my appointment?

You will need to bring in your completed intake packet, consent forms, previous assessment records, and any relevant medical/psychological records. Dr. Adams will take this information into consideration when completing your comprehensive report.

How can I prepare for my Psychological Assessment?

There is no need to study for the assessment. Here are some helpful suggestions:

Get a good night's rest the evening before your evaluation

Fees and Payments

Evaluations rates vary depending on the complexity of one’s background history and the reason for the evaluation. The following quotes are based on estimates of typical complexity though may require adjustments depending on the patient’s ability to participate and/or cooperate with the process.

A better estimate will be given after the initial consultation and prior to testing.

Psychological Evaluation for Surgical Clearance cost $1750 to $2950 per evaluation. The fee includes all evaluations, report preparation, and consultation.

Neuropsychological Evaluation for Transplant Surgery cost $2050 per evaluation. The fee includes all evaluations, report preparation, and consultation.

Cancellation Policy:

If you do not show up for your scheduled appointment and have not notified us at least 24-hours in advance, your account will be assessed a $50.00 cancelation fee. You will be required to pay the fee in full prior to scheduling another session. Please understand that by not providing us with advanced notice of canceling a session, we are unable to fill that spot.
